Pragmatic Free Trial Meta
Pragmatic Free Trail Meta is an open data platform that allows research into pragmatic trials. It collects and shares cleaned trial data and ratings using PRECIS-2 permitting multiple and varied meta-epidemiological studies that compare treatment effects estimates across trials with different levels of pragmatism and other design features.
Background
Pragmatic trials provide evidence from the real world that can be used to make clinical decisions. However, the use of the term "pragmatic" is not uniform and its definition and assessment requires further clarification. Pragmatic trials are intended to guide clinical practices and policy decisions, not to verify a physiological hypothesis or clinical hypothesis. A pragmatic trial should try to be as close as possible to actual clinical practices that include recruitment of participants, setting, designing, delivery and implementation of interventions, determining and analysis outcomes, and primary analyses. This is a major distinction from explanation trials (as described by Schwartz and Lellouch1), which are intended to provide a more complete confirmation of the hypothesis.
Truely pragmatic trials should not blind participants or clinicians. This can lead to bias in the estimations of the effects of treatment. Pragmatic trials will also recruit patients from different health care settings to ensure that the outcomes can be compared to the real world.
Furthermore, trials that are pragmatic must focus on outcomes that matter to patients, like quality of life and functional recovery. This is particularly important in trials that involve surgical procedures that are invasive or have potential serious adverse events. The CRASH trial29 compared a 2-page report with an electronic monitoring system for hospitalized patients with chronic heart failure. The catheter trial28, however, used symptomatic catheter associated urinary tract infections as its primary outcome.
In addition to these aspects the pragmatic trial should also reduce the procedures for conducting trials and data collection requirements in order to reduce costs. Finally pragmatic trials should try to make their results as relevant to actual clinical practice as they can by making sure that their primary method of analysis is the intention-to-treat approach (as described in CONSORT extensions for pragmatic trials).
Many RCTs which do not meet the criteria for pragmatism, however, they have characteristics that are contrary to pragmatism, have been published in journals of varying types and incorrectly labeled pragmatic. This can lead to misleading claims of pragmatism, and the usage of the term should be standardised. The creation of a PRECIS-2 tool that provides a standardized objective assessment of pragmatic features is a good start.
Methods

The PRECIS-2 tool scores an RCT on 9 domains, with scores ranging from 1 to 5 (very pragmatic). In this study the domains of recruitment, organisation, flexibility in delivery, flexibility in adherence, and follow-up received high scores. However, the principal outcome and method of missing data were scored below the practical limit. This suggests that it is possible to design a trial with good pragmatic features without compromising the quality of its outcomes.
